It's a sequel to a smaller work called "Neonomicon" (and an even smaller work called "The Courtyard" which I think is included in all recent printings of Neonomicon). Just be warned, if you're gonna pick it up: Neonomicon has a very controversial scene that is not for the squeamish.

I did a thorough reread of them all earlier this year (+ some other Alan Moore Lovecraft stuff), and there really is a lot of depth to them. If you're well read with Lovecraft, there'll be a lot of references (especially in Providence...).
